Rejuvenescence. Generalities.
One of the aspects of cell-based therapy in esthetic or cosmetological medicine is using committed stem cells culture from the fibroblasts which belong to the family of basic connective tissue cell forms, for face and body skin rejuvenation. The connective tissue, consisting mainly from fibroblasts, plays a great role in both organism development and insenescence. It is known that one of the main reasons of skin aging is decreasing of skin fibroblasts potential to mitosis. As a result, the amount of fibroblasts in skin becomes smaller, their activity is reduced, and their ability to synthesize intercellular matrix, collagen and elastin fails; the synthesis of hyaluronic acid in dermis and epidermis also decreases along with acceleration of its breakage under the influence of various factors. At the same time, the dermal layer thickness and skin humidity decreases, because of total skin blood supply deterioration and water saving systems dysfunction. As soon as the stem cells potential becomes poor, a person becomes aged, the tissue regeneration stops
and the risk of incipience of various acute and chronic diseases including oncological ones increases.
With the age, the amount of skin fibroblasts (specific cells that synthesize collagen, elastic and other intercellular matrix components) decrease. When exposed to natural aging processes and affected by environmental aggressive factors, the skin dermal layer becomes thinner, the skin moisture rate descends, the skin ages, loses its flexibility and elasticity, skin dryness and peeling appear, and wrinkles and creases are formed.
The essence of advanced theory of rejuvenation with stem cells is based on the principle of regular and targeted regeneration of aged or affected dermis and epidermis areas by means of endogenously deposited bone marrow stem cells. The pictures show the stem cell of fibroblast type that was obtained in vitro according to the original technology together with pluripotent final culture differentiation towards morphologically identified cells.
Migrating by means of directed homing through the microcirculatory system of dermal blood capillaries, fibroblasts themselves find the exact area of dermal tissue where they are needed most. After entrenching themselves in this area, stem cells not only begin the intensive mitosis regenerating the dermal tissue, but also stimulate the mitosis of the tissue’s own cells, thus prolonging theirs vital potential. Migrating to the impaired place, fibroblasts produce the new dermal connective-tissue matrix and synthesize growth factors and hyaluronic acid; they restore microcirculation, enhance metabolic processes and blood circulation. A young cell while contacting with the aged one induces reprogramming of the aged cell by genetic information transfer. As a result, the retardation of skin aging due to intensified physiological recovery
mechanisms takes place. The skin color turns healthy, and becomes velvety and elastic by touch; the skin dryness and peeling disappear; deep wrinkles become smoother and small lines vanish completely. As an implant, stem cells likely pass the cycle of transformation into fibroblast type and launch a wide range of biochemical reactions intended for regeneration of affected skin elements. The aspirate of red bone marrow taken from sacropelvic or femoral bones of gilts or the patient, who undergoes a cosmetological procedure, can be used as a source of primary cell material. During the process of cultivation according to the original technique, the final pluripotent bone marrow stem cells culture differentiate into morphologically identified cells under the influence of different factors.
Along with original biotechnology of fibroblast-like cells generation, we have developed the innovative cosmetic and dermatologic compositions produced as soft dosage form (SDF) and soft cosmetic form (SCF, cream/gel). Both forms are oil-in-water emulsions (o/w emulsion) with different content of oil phase (mineral components and lanolin); besides complex emulsifier, they additionally include the low-molecular hydrophilic non-aqueous solvent (LMHNAS) of propyleneglycol- or glycerine-type, and contain a stable dispersion of substance of biological genesis (SBG) for the achieving of synergism effect between different agents. The mammalian (gilts’) pluripotent bone marrow stem cells have been used as SBG. Furthermore, SDF or SCF contain preservative agent, fragrance (rosemary essential oil), and polymer thickener (cellulose or acrylic acid derivatives). Due to the using of LMHNAS, the background for active diffuse transport of biologically active substances (BAS) into the cell through transmembrane protein complexes accompanied by progressive decreasing of buffer osmotic pressure relative to osmotic pressure inside the cell have been created (Fig. 1). The using of LMHNAS has set the
saving of stem cells vitality at the initial stage of their introduction into SDF or SCF, followed by “sparing” release of BAS along with destruction of cell structure without any previous cell mass homogenization (Fig. 3). At this forming phase, SDF or SCF is saturated with soluble stem cells components transferred into buffer solution during the process of stem cells homogenization. Figure 2 clearly shows the diffusely scattered stem cells with size of about 12.5-20.0 μm in swelling phase due to the osmotic equilibrium shift induced by the presence of LMHNAS molecules of propylene glycol-type in aqueous phase. The biologically active substances transported by LMHNAS stimulate stem cell migration and induce mature dermal stem cells to their regeneration and directed differentiation into fibroblast-like cells able to synthesize the intercellular matrix, collagen and elastin. This process is additionally stimulated by hyaluronic acid synthesis in dermis and epidermis. Therefore physiologically there is influx of additional stem cells in dermis and epidermis from circulating blood, stimulation of own stem cells situated in the skin, and reprogramming of aged dermal cells.
Rejuvenescence. Conclusion.
The pluripotent stem cells from mammalian bone marrow cultivated in vitro work as an implant compensating the actual defect of dermal tissue. Stem cells facilitate progressive regeneration of dermal supporting structure at the area of tissure “defect”. The uniqueness of technology is that patient is inserted either his/her own stem cells or consanguineous ones. The cell implant, which compensates the actual defect of dermal tissue, becomes the source of biologically active substances which are necessary for normalization of tissular metabolism. The patient does not have any problems related to immunological (tissular or histological) compatibility and rejection of transplanted material . The overdosage risk is also excluded since unreclaimed stem cells are deposited in bone marrow replenishing its depleted deposits. The process of skin aging prevention and smoothing, for example, of skin wrinkles, is performed in the most natural way. Skin becomes more shiny and elastic and its appearance turns healthy.
Therefore, our studies suggest that stem cells insertion into problem areas of the skin is followed by:
The using of LMHNAS new formulation in combination with stem cells homogenate taken from mammalian (suckling pigs’) bone marrow enhances the cell metabolism and increases the concentration of water and nutrients inside the cell; young cells begin to proliferate, giving growth to the new or displacing the old and “affected” cells. All the above suggests that active enchantment of anti-inflammatory and regenerative effects are induced due to the presence of LMHNAS in formulation of soft dosage (SDF) or soft cosmetic (SCF) forms. One of the specific characters of synergic effect of SDF or SCF active principles is the fact that the form itself can serve as substrate for cultivation of pluripotent mammalian stem cells, for example, those stem cells that redound to skin regenerative processes on sites of extensive burns. Tests performed with participation of approbators (n=30; women between 38 and 56 years old, weight 65–85 kg) and using of experimental SDF and SCF together with biologically active substances (BAS) of stem cells homogenate taken from mammalian bone marrow, have shown the externally reliable similar effects, including anti-inflammatory, anti-herpetic and regenerative effects, which can be named as typical.
The visual examination of skin “problem” areas (wrinkles, dryness, intertrigo, peeling etc.) has revealed healthy colour of skin and increasing of turgor of skin tissue; the skin has become velvety and elastic by touch, both dryness and peeling have disappeared, deep wrinkles have become smoother and small lines have vanished completely.
